Timeline of Events

MORNING OF THE INCIDENT

Four males (including a 16-year-old apprentice) were performing maintenance operations while working on top of the Anaerobic Digester (AD).

INCIDENT

At 11:22 AM – A massive explosion occurred at the anaerobic digestor, resulting in fatalities of the four men, as well as an injury to an additional worker, who was working near the AD.

IMMEDIATE RESPONSE

Emergency services were promptly dispatched to the scene and nearby residents were evacuated due to concerns about potential release of H2S.

POST-INCIDENT INVESTIGATION

Health and Safety Executive (HSE) and local authorities conducted a thorough investigation that initially found that the explosion was triggered by a mix of chemical substances, but the root cause of the incident was yet to be determined.

Important Notes

Root Cause Identified

The investigation revealed lapses in safety protocols, including:

  • Improper storage practices.
  • Inadequate risk assessments.
  • Failure to implement effective hazardous area classification.
  • Maintenance issues and equipment failures.

These deficiencies directly contributed to the explosion and its tragic consequences.

Lessons Learned

OPERATING PROCEDURE AND SAFETY CULTURE:

A stronger safety culture at the Avonmouth facility could have helped identify, assess, and mitigate the risks. Regular equipment inspections and adherence to safety protocols are essential.

HAZARDS AND RISK MANAGEMENT:

The explosion highlights the need for comprehensive hazard and risk assessments, especially when working with volatile substances in confined spaces.

MECHANICAL INTEGRITY:

Maintenance issues and equipment failures were contributing factors. A robust Mechanical Integrity program could have identified and addressed these issues before the incident.

REGULATORY OVERSIGHT:

Following the incident, HSE issued a notice to Wessex Energy Group highlighting urgent safety deficiencies. This led to a nationwide review and stricter regulations for similar facilities.

COMMUNITY AND ENVIRONMENTAL PROTECTION:

Effective emergency response plans and communication strategies are critical to safeguard public welfare during industrial accidents.

REPUTATION MANAGEMENT:

The incident negatively impacted public perception of anaerobic digesters and Wessex Energy Group. A commitment to process safety and transparency is vital for rebuilding trust and ensuring sustainable operations.
